The Canadian Alliance of Community Health Centre Associations (CACHCA) and the Association of Ontario Health Centres (AOHC) have produced a resource to help interprofessional teams work more effectively together. Building Better Teams: A toolkit for Strengthening Teamwork in Community Health Centres shares the knowledge and skills teams need to be successful. Incorporating evidence from the experience of Community Health Centre teams, current literature on effective interprofessional teamwork, and the skills needed by all high performing teams, the resource is a useful reference for primary health care teams of all kinds.

The Association of Ontario Health Centres received funding from the Primary Care Health Transition Fund (PHCTF) to study Best Practices in the Evaluation of Interdisciplinary Primary Health Care Teams. The project was a partnership between AOHC, the University of Toronto, the Institute for Clinical Evaluative Sciences (ICES), the University of Western Ontario and Lakehead University. The research sought to define, measure and produce recommendations for improving effectiveness in interprofessional teamwork. This document if the final report from the Research Project.

What is the Indicators Project?
The CHC sector in Ontario has more than three frameworks that identify indicators to be measured and reported on by our sector. Namely:
1. The Sector Strategy Map
2. The Program Logic Model and Evaluation Framework
3. The Accountability Framework
Our accreditation system (BHO) also identified Indicators for the sector.
In 2005 AOHC initiated the Indicators Project, to ensure that each of these frameworks were coherent and mutually reinforcing. The first phase of this work has been completed. The work will continue in 2007-8 through the resolution of some outstanding issues and the development of a core set of indicators for the sector. For more information about next steps, please refer to the memo below.
What decisions have been made?
There have been 3 documents produced to date. Each of these documents have been approved and accepted as guides for our future work. They are as follows:
Connecting the Dots – This proposes a limited number of domains, which are common to the 3 frameworks mentioned above, as well as the MOH System Strategy map and the MOH Primary Health Care Strategy Map.

Options for Consensus and Evidence Based Indicators – This document provides a menu of possible indicators for each of the domains identified in the first document. All of the indicators included are both evidence based and selected through consensus processes.

Proposed Results-Based Logic Model to Support Performance Measurement, Accountability and Evaluation – This document provides a revised Logic Model for our sector. The new logic model is consistent with both the Strategy Map and Accountability Framework.

The purpose of this document is: 1) to clarify what effectiveness means for CHCs; and 2) to present a framework that will help the AOHC and individual CHCs to develop research projects that will assess their effectiveness. Citizen engagement and community governance is believed to have many benefits for health and health care. Enhanced quality of health care, improved individual and community health outcomes, better accountability, and more efficient use of resources are key dimensions of health and health care where engagement of citizens can have a positive impact.
This review was commissioned by the AOHC to gather information and evidence on the concepts of community engagement and community governance within the context of regionalized health systems. The purpose of this literature review was to present evidence on the positive benefits of citizen engagement and the value added by inclusion of citizens in local organizational community governance in health care planning and decision-making.

The effect of regionalization on local community engagement and community governance is an area of particular importance to CHCs in Ontario, as LHINS have now been established. This scan was commissioned by the AOHC with the purpose of gathering information that would contribute to a greater understanding of the effects that regionalization has had on CHCs from regions across the country.
The results of this scan are intended to contribute to discussions and debate about community governance and engagement as Ontario begins to implement health care change.

The Association of Ontario Health Centres (AOHC) is pleased to present the final report HIV/AIDS Prevention, Care, Treatment and Support: Best Practices for Community Health Centres.
Our research has documented the HIV/AIDS services that Ontario CHCs are providing to their communities; identified the successes and challenges of that work; and clarified the role that the CHC sector plays in HIV/AIDS care in Ontario. Further, the project has demonstrated the strategic role of CHCs in addressing the HIV/AIDS epidemic in Ontario and has provided clear recommendations for improving the integration of CHCs into the HIV/AIDS Strategy for Ontario to 2008. It is our hope that this document can be used to guide the next steps in provincial collective action towards the development of HIV/AIDS programs in local centres.

Cost effectiveness literature review -- In December 2004, the AOHC commissioned a report on the current state of the academic literature concerning the cost-effectiveness of CHCs. A growing set of assumptions about CHC cost-effectiveness required a definitive review of what evidence actually existed. Based on this review, suggestions for future research for the CHC sector have been made.
The actual results of CHC cost-effectiveness research are encouraging. Although the research is dated, CHCs perform well on both the cost and effectiveness fronts.

Early Years Best Practices Report -- In the winter of 2003, the AOHC in conjunction with the Community Health Centre Program, Ontario Ministry of Health and Long-Term Care initiated a provincial evaluation project for the Early Years projects implemented by community health centres (CHCs).
An overarching goal of the project was to support CHCs by providing them with resources that would assist in the planning, implementation and evaluation of Early Years activities. The project focused on validating and supporting the work of community health centres in parenting and early childhood development.
The final report, contains the following chapters:

Research Readiness in Community Health Centres (CHCs) & Aboriginal Health Access Centres (AHACs) -- In June 2001, the Research Advisory Sub-Committee of the Association of Ontario Health Centres produced a manual to provide Aboriginal Health Access Centres (AHACs) and Community Health Centres (CHCs) with tools to support "research readiness: in their centres.
The manual contains guidelines, prototype policies and procedures, suggestions on structuring a research committee, and specific tools that may be adapted and used.

Community Health Services - What Constitutes a Satelite? -- In January 2001, and revised in May 2001, the Alder Group Inc prepared a discussion paper on satelites. The purpose of the paper is to provide a set of clear definitions that can be used by CHCs, the Ministry of Health and Long Term Care, the AOHC, District Health Councils, and emerging groups to plan for and implement satellite and other off site services.

Building on What We Know: Research by Community Health Centres -- A database was developed to collect information on research projects in which AOHC member centres had been involved over the past 5 years. As of June 2008, a total of 41 research project were collected in the databse, which is intended to be a searchable, publicaly accessible source of information. This report presents an analysis of the projects in the database, sorted according the areas of focus of the research.
