As of April 2006, administration of Ontario's health system has been divided between 14 regions known as Local Health System Integration Networks (LHINs). The number of Community Health Centres (CHCs), Francophone CHCs, and Aboriginal Health Access Centres varies significantly within each region or “LHIN”.
The above map identifies the geographic areas covered by the 14 LHINs. In many instances it may be challenging to identify which communities are located within each LHIN. Communities within the Greater Toronto Area are distributed among 5 LHINs. To identify a centre near you, you may search by LHIN, or by entering a postal code in the option provided above.
Separate links are provided at the bottom of the page to lists of all Francophone CHCs and Aboriginal Health Access Centres.
